Referring a client for home care
We welcome referrals from social workers, discharge planners, case managers, care coordinators and health care providers serving clients in Ramsey County.
How to refer
- Call our office during office hours to tell us about the client's general support needs and location.
- We check fit and availability and tell you honestly whether we can help.
- We contact the client (or their representative) to talk through services and next steps, with their consent.
- We keep you informed as appropriate and as the client permits.
Protecting client privacy
Please do not email or text client names together with health information, diagnoses, insurance numbers or other private details. Call us, and we will arrange a secure way to share the information we need.
